The drug has an analgesic and antipyretic effect, mainly by inhibiting the activity of prostaglandin cyclooxygenase in the CNS It is well absorbed from the gastrointestinal tract, reaches its maximum concentration in the blood after about 20-60 minutes, penetrates into body fluids and internal organs, in about 25% it is bound to plasma proteins. T 0.5 is 2-3 hours. It is metabolized in the liver, inducing microsomal enzymes there; excreted in the urine in the form of glucuronates and sulfates and after conjugation with cysteine. In patients with renal failure, conjugated paracetamol accumulates in the blood. After high doses, N-acetyl-p-benzoquinone is produced in the body, probably responsible for causing liver damage.

Efferalgan rectal suppositories Indications:
Symptomatic treatment of pain and fever.

Contraindications:
Hypersensitivity to the drug (rare), congenital deficiency of G-6-PD and methaemoglobin reductase. Caution should be exercised in patients with bronchial asthma allergic to acetylsalicylic acid and in the case of repeated doses in patients with severe hepatic impairment. It should not be used in children under 3 months of age.

Adverse reaction:
There are isolated cases of haematological disorders with long-term use (thrombocytopenia with symptoms of thrombocytopenic purpura) and allergic skin reactions (urticaria, erythema, itching, rash). When used in high doses or for a long time, it may impair liver function.

Interactions:
Paracetamol does not alter bleeding time and platelet aggregation in healthy and haemophilic patients. Used with drugs that inhibit the breakdown of paracetamol in the liver (rifampicin, antiepileptics, some sleeping drugs) increases the risk of liver damage, with lamotrigine or chloramphenicol - it lowers their concentration in the blood. Cimetidine, metyrapone, probenecid, antipyrine, salicylamide, isoniazid, ranitidine, propranolol - prolong the half-life of paracetamol in the blood.



Dosage Efferalgan rectal suppositories, fever in children:


In children under 3 months of age paracetamol can only be used on the express recommendation of a doctor. Recommended doses rectally for children over 3 months of age: 10-15 mg / kg bw 4 times a day, in selected situations every 4 hours - this dose can be used for up to 3 days. Do not exceed the daily dose of 60 mg / kg bw Adults and adolescents over 15 years of age. 600 mg 1-4 times a day. Do not exceed a dose of 3.0 g per day. There should be an interval of at least 4 hours between doses. In case of severe renal failure (creatinine clearance below 10 ml / min), the dose intervals should be at least 8 hours.
